Toro 06 LX460 - Bolt / Spacer Falling Out.

So as the title indicates, I have a problem with my Toro. The Bolt and Spacer are falling out now this year as the drive belt is engaged. My guess is the belt is loosening the bolt/spacer when I am engaging the double idler pulleys. The Nut/Thread appear to be built into the frame, unless I need to take the shell off to replace this part. The bolt will not tighten up 100% either. I'd guess it gets to about 40% torgue and then spins free again when tightening. Any advive/thoughts greatly appreciated!

IF it is the variable speed double pulley. Otherwise give us more on which screw and washer is coming loose.

11 is Bolt, 29 spacer, and 13 is what appears to be a flange nut attached/built to frame.

Maybe all I need to do is take the fender off. Just don't want to mess with the tower if at all possible.

Replace the flange lock nut with a new one as nylon part is probably badly worn and no longer holding.
