MTD 30 inch cut

Hi can anyone explain why this 30'' MTD mower keeps throwing the cutter belt , it is a 1338560 model , Replace the belt all tensioned up look to be running straight used it once come off again . This old girl has a crankcase oil leak just wondering if the shaft is out of balance???

Does it come off when your cutting ( pto on ) or when you disengage the pto?

I have not used it but what was told it must happen when disengaged the pulley on the engine shaft has a guard on it , As it has only 1 deck spindle should there be a guard on there too ??

The guard should have guide pins on it to keep the belt on when when you disengage the pto and the belt becomes loose. There should be a guard over the pulley on the deck. And be pins or a bar almost touching the engine pulley
