Homelite UT13126 Fan repair

Hi there, I have a Homelite UT13126 my wife got off of CL last year. Recently replaced the battery, and then decided to sharpen the blade. I put it all back together, and there might be something I missed with the blade assembly. First, I must have overtorqued the nut that secures the blade assembly, and the blade insulator failed. I managed to get another insulator, and got it to hold, but it seems the fan is spinning loose on the spindle, as it looks like it's melting a hole into it (and I can hear that something is wrong and shut it off). Am I missing a part here? The photos show the fan where it meets the spindle, and the other picture the spindle. Turned out the spindle guard (#39) was inverted. Must have been the previous owner - wasn't easy to remove and flip over, but now it works great.
