Sounds like worn valve guides letting too much oil past them. Why it makes such a big puff of smoke when you first start it is the valves and guides haven't heated up and have a lot of clearance. That oil exposed to high vacuum starting up will suck that oil right past the valve guides through the valves and into the engine and gets burned in a big cloud of white or blue smoke. And could also be some ring wear on the piston as well. If it's not burning that much oil or fouling plugs and you can live with it, I'd postpone surgery maybe till this winter as a project! Hope this helps.
