I know this sounds like a dumb question. Does the blade go above or below the spacer? I put the blade below and I have too at 4.5 just to keep from scalping the grass.

The blade goes below the spacer. There is something wrong with the deck height adjustment if you are having to cut on 4.5" to keep from scalping. Make sure the blades aren't canted or something unusual like that.

Check your deck height with a ruler. I have a Turf Tiger and it was a little out of adjustment, but nothing like that. The inch markings are pretty accurate to the blade height. Most of my cutting is at 2.75 inches with some early season yards at 2.5" and some overgrown ones at 3".

If you have a manual, it will give good directions on adjusting the deck height and level.

One thing I have noticed is the blades are below the bottom of the deck in front and when you turn them to the back, they are about a qtr inch above the bottom of the deck. I have not moved any of the deck adjustments.

Is there a chance that the blades are bent? The spindles on a Scag are so massive that I can't imagine that they are bent in any way.

If the blade(s) are not bent, check to make sure that there isn't something getting between one side of the spindle and the blade/spacer when it is installed.

Something is causing that blade to be canted. Either the blade is bent or it is not bottoming out completely on the spindle.

The blade ends should rotate just above the bottom edge of the deck, never below it.

There is a custom cut baffle across the front underside of the deck which is adjustable. The position of this baffle could be from above to below the blades when they are positioned directly forward.
I know of several who have removed their decks but for no good reason and screwed up the adjustments when the deck was reinstalled.

I lifted mine up today for a blade inspection. The blades on my machine look in good shape. I took a picture of how I lifted it up.
I thought of buying a special jack for it, but this works fine and didn't cost me anything!
To answer your question as others did,
The bolt head, blade then the spacer from bottom to top....

Ok, I have mowed for a couple of weeks and have been trying to figure out why I have to mow with my deck height set at 5 inches. That gives me the best cut. The problem is the blades are even (actually, below the bottom of the deck).
