I was filling up a Gator at work one day. When the nozzle shut off, the hose split at the pump and sprayed gasoline 25' to 30' across the parking lot. Luckily for me the split was on the opposite side of the pump from where I was standing, and I was able to shut the pump off without getting into the spray.
That would be Kentucky. The Plant Shift Supervisor wasn't too happy either. Of course, fire services got to play with their trucks and other equipment, so they were having fun.

More gasoline is spilled annually than what flowed from the Exxon Valdez.
