fx850v oil cooler nipple just came out of the block???

What is suposed to hold the nipple in? The nipple was still clamped to the oil cooler line. I am wondering if this is a comon flaw? Or if anyone else has had this happen? I sure do not want it to come out again. I believe I got it shut down before any engine damage.
