problems with stihl km130 kombi system

having issue with my km130 kombi system on fs-km brush cutter attachment keeps flying off shaft no matter how hard i try to lock it could it be problem with steel coupler that is supposed to hold attachments in place, i have not seen anyone as of yet with this issue please advise
