My 19.5 Briggs started running bad and I found the exhaust valve adjustment nut was not tight and the rocker arm very loose. I adjusted the valves and it ran fine for a few minutes. Now it sounds like crap again. Do I have a cam going bad?

If it ran fine a few minutes I doubt the camshaft is the problem.
Well did you check the rocker stud? They do come loose. They need to be torque to 150 in/lbs and sometime it even take blue Loctite on the threads.. And the setting screw or locking nut torqued to 45in/lbs.
Also make sure the push rod is not getting bent.
